This application form is designed for Professionals and Practitioners only who wish to refer families and individuals for food support to Dunbar FoodShare. Our food bank provides a referral service only for people in crisis. Please complete all sections of the form, otherwise you will be unable to submit it.

Estimated time this crisis may last – please note that we usually allow up to six weeks support within any six month period. Our ethos is to work with referrers to be flexible in crisis and support people without creating dependency. Once we have supported a client for a number fo weeks we may, in exceptional circumstances extend the period of cover:
